Documents & Requirements

Documents Required :

1. Latest original Form BE/B (husband & wife) with combined minimum income of RM36,000.00 per annum for Indonesia, Combodian and Vietnam maids. RM60,000.00 per annum for Philippino maids.
2. Latest Original EPF statement if he/she is without Income Tax Returns Forms.
3. Income tax receipt for BE/ B if any tax payable at the end of the year (husband & wife)
   Self-Employed:
   • Sdn. Bhd. – Copy of Form 9,24 & 49 if appointed Director.
   • Sole proprietor – Copy of Borang B & Borang D
4. Latest 3 months photocopy pay slips (husband & wife).
5. Original Electricity / Telephone Bill (or any other utility bill)- if address differs from NRIC.
6. Photocopy of NRIC – front & back (husband & wife).
7. Passport size colour photo – 2 pieces each (husband & wife).
8. Photocopy of marriage certificate.
9. Children Birth Certificate below 15 years old.
10. Check Out Memo (COM) if maid have sent back to the origin country (only applicable for those employer has applied maid before).

If you are applying a maid to look after old/sick parent the document to be submitted are:

1. Photocopy IC of above parent.
2. Doctor’s letter (original) from hospital.
3. Birth certificate (photocopy) as proof of relationship between employer and old/ sick parent
